SEURO - Scaling up Proactive Digital Integrated Care
SEURO (Scaling EUROpean citizen driven transferable and transformative digital health) is an EU funded Horizon 2020 project (grant no. 945449) that will span a year 3 period from 2021 to 2024. Caredoc is proud to be one of 12 partners across 5 EU member states in the SEURO consortium, supporting research into the effectiveness of digital health interventions, led by the Trinity Centre for Practice and Healthcare Innovation at Trinity College, Dublin.
The project aims to scale the ProACT platform to advance home-based self-management and integrated care for 720 older European adults with multimorbidity using wearable monitoring devices at home, while providing insight into overall health system performance.
The project will convene 2 Effectiveness Implementation Trials over two 6-month timeframes and is seeking participants who meet the following criteria to take part –
People aged 65 and older with 2 or more of the following conditions:
Diabetes
COPD/Emphysema/Chronic Bronchitis/Chronic Asthma
Chronic Heart Disease/Coronary Artery Disease/CVD
Congestive Heart Failure
